Eligibility & Application Information

The Wayne Township Trustee provides emergency assistance for individuals who meet the eligibility requirements. Individuals 18 years of age and older may apply for assistance. No person can be seen for assistance without an appointment. Please call 317-241-4191 to make an appointment and receive further instructions. Please note, clients are seen Monday through Friday, 8:30 am to 3:00 pm by appointment only.

Application Procedure:

All applicants applying for township assistance will be processed according to written uniform standards, without regard to the applicant's age, sex religious creed, national origin, political belief or physical or mental handicap.

 

Persons that may qualify for other benefit programs will be required to make a formal application with those programs.

 

To determine eligibility, the Trustee will consider resources and assets of the household such as cash on hand, checking and savings accounts, certificates of deposit, stocks, bonds, mutual funds, proceeds of grants, student loans or stipends, recreational vehicles, and any other resources.

 

The township may require households requesting township assistance to allow an inspection of their living quarters. The inspector will assess the unit's compliance with the minimum standards of health, safety, and decency, and will determine the amount of rental assistance per trustee standards. Determination will be made within 72 hours of the initial application. Holidays and weekends are excluded.

 

Wayne Township will accommodate persons who are unable to physically come into the township office to apply for township assistance. Arrangements for home visits may be made by calling the office and making an appointment. Forms may be accessed on this page of the website. When you are making your appointment, please ask what forms you need to print and complete for your appointment.

 

What You Need to Bring on Appointment Day

1.) Personal Identification for all adults in the household 18 years and older.  

  • Acceptable forms of ID are current and valid driver's licenses, state identification cards, school ID cards, or employee ID cards

2.)    Social security cards for everyone in the household. 

  • Signatures are required on all adults’ social security cards.  

  • Proof of applying for a duplicate card with the Social Security Office is acceptable but must show name and social security number.  You must request the second page that will show your social security number.

3.)    Non-U.S. Citizens must present: Visa, Green Card, Permanent Alien Card or some proof of your legal status to be in the United States

4.)    Proof of current residence within Wayne Township

  • All of the following are acceptable: lease or mortgage documentation, and all utility bills

5.)    All adults (18 years or older) residing in the household are required to sign the application.

6.)    Proof of household income for ALL Adults (18 years or older) in the household, for the past 30 days.

  • This may include: Social Security, AFDC/TANF award letters, child support printouts, pay stubs, statement from employer, a notarized contribution statement from any persons providing financial support, printouts of all bank account(s) transactions, unemployment compensation.

7.)    Wage printout from Work One for each adult in the household (must be dated within 30 days of your appointment.)

8.)    All paid receipts for the past 30 days

  • Paid receipts can be:  cancelled checks, money order receipts, notarized statements (such as for paid child care), utility bills stamped by utility company, printouts of bank transactions (if states the paid party), and proof of all transactions from prepaid debit, EBT and EPPI cards.   

9.)    If an adult in the household is under doctors’ care, we will need a current doctor statement as to their ability to work or if they have any restrictions.

10.)    Proof of vehicles owned, leased, used 

  • Notarized statement, contract and current registration.

11.)    February through June – Required: proof of filing and status of income tax returns.  

  • All receipts showing how your refund was spent

 

Tips for Applying

  • Be on time, be patient and be honest

  • Bring all information requested and more if you can

  • Keep all appointments with other agencies

  • Keep all receipts and pay stubs
